Skilled Nursing:
Affectionate Heart, Inc. will assign you a highly competent nurse to provide you with top-level skilled services. Our nurses will be responsible for the creation and implementation of your treatment plan. We will make sure that everything you need will be attended by our staff.
Skilled Nursing includes but not limited to: Wound Care, Treatments and Injections, Supervision of Medications, Rehabilitative Care, Respiratory Care, IV Treatments, Family-Patient Health Teaching, Dressing Changes, Drawing Blood and Lab Work, and Diabetic Training.
Physical Therapy:
Affectionate Heart, Inc. Our physical therapists (PTs) and physical therapy assistants (PTAs) provide individualized patient education in transfer techniques and the use of adaptive equipment such as wheelchairs, walkers, crutches and Hoyer lifts. They also provide instruction to our patients and caregivers on long-term exercise programs designed to improve the patients’ level of independence. Occupational Therapy
Occupational Therapists from Affectionate Heart, Inc. will help you or your family member in dealing with injuries or disabilities after a work related incident. The goal is to restore or enhance your self-care skills – or if possible, to restore your work skills so that you can resume your job post upon recovery.
Speech Therapy
We learn how to express ourselves, we learn how to communicate. However, when we have a health condition that prevents us from relating to friends and family, the quality of our lives may be diminished.
It may be the effects of a stroke or an involuntary stutter that you’ve had to deal with your whole life – at Affectionate Heart, Inc. we offer Speech Therapy services to help you enhance or regain your communication skills.
Medical Social Worker (MSW)
To help you get through the mental and physical stress of dealing with your loved one’s medical condition, our medical social worker will provide you with assistance and resources to lessen any burden your family has. You can talk to us about your needs; our staff will be with you every step of the way. We help you with: Education Regarding Alternative Living Arrangements, Case Management, Advance Directives (Power of Attorney for Health Care or Living Will), Community Resource Planning and Coordination, Short-term Therapy, Crisis Intervention, Financial Assessment, Counseling with Life-style Changes, Teaching Regarding Coping with Loss and Change, and Grief counseling.
PAS
Affectionate Heart, Inc. wants you to stay healthy and ensure your quality of life through our personal assistance services. Our staff will take care of your needs and see to it that you are comfortable and safe. You can rely on them to be of great assistance to you with Bathing, Dressing, Meal preparation, Feeding/Eating, Exercise, Grooming, shaving or Oral care, Routine hair or Skin care, Toileting, Assistance with self-administered medications, Transfer, Cleaning, Laundry, shopping, and escorting.
